When I'm vertical, I cough less (good thing) and am sleepier (also good)
When I'm horizonal I cough more (bad thing) and am wide awake (also bad.)

Don't get it. Although at least the yellow sludgy cough medicine is doing a good job at knocking out the cough, even if the sleepiness it causes seems to not go away.

Unless the exhaustion is either the lack of sleep catching up with me or the lack of air catching up with me. (peak flows still very low, but I can't figure out what else to do. They started dropping very quickly after the nebulizer treatment last night-within 45 minutes they were back down to where they were in the AM. I figure a few days of the medication and then maybe I'll worry.)

By any chance is the cough medicine Tussionex? I had that prescribed this last spring when I had a slight pneumonia. It's a fairly potent but effective cough medication that has Vicodin.

This probably a combination of both. I hope you feel better soon.

Don't try to lie down sweetie. You need to fix yourself a nest where you can sleep in a vaguely sleeping position. Pile a bunch of pillows on the bed. Use extra blankets if you dont have enough pillows but dont lie down. A 45 degree angle is enough and should still be comfy enough you can nap.
